Question 1: B (indicating preference to receive both primary and secondary results)
Questions 2 through 11: “No” with the exception of “NA” on either 9 or 10.
Questions 12 through 15: “Yes”

Interpretation:
Patient or participant only wants results relevant to reproductive decisions, and no results indicating his or her own genetic risk.

Analytical Plan:
Analysis should include all genes relevant to reproductive decisions including those relevant to carrier status, as well as those indicating predisposition to childhood-onset diseases that the patient or participant clearly did not develop (such as genes predisposing for autism). Genes only relevant to the patient’s own risk should be omitted.
